Upon completion of this form an authorization number will be issued and returned to you. Unless specified, it is the responsibility of the customer to pay inbound transportation charges for failed units (warranty or out of warranty), and the responsibility of Powerwave Technologies, Inc. to pay outbound transportation charges for the repaired unit. Please make a copy of this form for your records and be sure to include a copy along with your return.
